A member of the Chicon 2000 committee will be assigning all Hyatt Regency Chicago suites for the convention. Remember, this is a volunteer position, so please be patient.

If you prefer to send paper, please print this form, fill in the blanks, and mail it to Chicon 2000, attn: Suite Coordinator, P. O. Box 642057, Chicago, IL 60664 or fax it to 312-946-3779.

All Hyatt Regency Suite reservations will be processed by the committee, then submitted to the hotel for credit check. You will receive your confirmation from the committee. We will also be sending you a packet including party rules, hotel requirements, and information on local amenities. To change your reservation, including cancellation, please send email to suites@chicon.org or use the address or fax number above. Please be sure to get your change request in well before the first night of your stay or your credit card will be charged for your first night's rent.

A damage waiver and/or a deposit for your suite may be required by the hotel depending upon the quality and location of your suite.

Both the committee and the hotel will do our best to insure that your stay is as pleasant as possible.

The deadline for suite reservations is 03-Aug-2000.

There will be a minimum charge of $500.00 for furniture removal. The hotel imposes this charge to cover the cost of moving furniture to off-site storage. Because Chicon is occupying almost every square inch of the Hyatt, on-site storage is nil.

Please note: These prices do not include 14.9% tax, which will be added to the room rates.
Parking is $13 per day with unlimited in/out privileges.
Additional sleeping rooms added to the suites are $102 per night each.
Extra refrigerators are $20 per day.
Rollaways incur a $20 one-time charge.

* The "Astor" and "Monarch" suites have the bedroom in the parlor, not multiple rooms.

** Requires personal approval by the hotel's general manager.

NP - No parties will be allowed in these rooms.
